incident-learning
From the author
Convert operational incident and near-miss evidence into durable product, engineering, test, evaluation, and governance improvements with verified closure. Separate observed facts from causal hypotheses and unresolved uncertainty; map follow-up work across code, tests, skills, operations, product, and governance; track ownership, verification, and closure for every finding. Do not use to assign blame or produce a generic postmortem template; do not close learning because tickets were created — require evidence the intended change occurred.

- incident-managementcbrock84/headcountRuns an operational incident from detection to closed action — declaring it and naming a commander, separating the people restoring service from the people communicating, keeping a timeline as it happens, deciding when it is over, and running a review that produces a small number of changes someone actually completes. Use this to set up an incident process, run one, work out why the same failure keeps recurring, or fix a review practice that generates findings nobody closes.Engineering★ 2.1K
- product-lifecycle-learningmagnus919/agent-skillsCompare intended product outcomes against observed results to close the launch-to-learning loop: collect post-launch evidence, distinguish expected from observed from uncertain from inferred claims, update assumptions, assess feature health, and choose among continue/improve/harvest/pivot/pause/retire — including retirement lifecycles with deprecation, migration, customer treatment, and retained reusable learning. Do not use for incident postmortems or root-cause analysis (routes to incident-learning or site-reliability-engineering); do not use for analytics instrumentation or metric dashboard design (routes to product-analytics-and-measurement); do not use arbitrary thresholds as universal retirement rules — decisions require human judgment and context.Data★ 147
- incident-responseanthropics/knowledge-work-pluginsRun an incident response workflow — triage, communicate, and write postmortem. Trigger with "we have an incident", "production is down", an alert that needs severity assessment, a status update mid-incident, or when writing a blameless postmortem after resolution.OfficialWriting★ 32.5K
- continuous-learningericrisco/rsc-harnessUse when a mistake, correction, or surprise taught the workspace something that must stick — a retro or postmortem, the same agent error corrected twice, a resolved bug's root cause, scattered notes-to-self — and route that lesson to the durable surface that fires next time. NOT a forward choice with alternatives (that is `decision-records`).Productivity★ 255
- incident-validation-correlationzero-lab-ai/skillsCorrelates supplied sanitized incident observations with separately approved isolated defensive-validation receipts across prevent, observe, detect, contain, and recover. Use for defensive learning comparison without live response, hunting, technique design or execution, detection authoring, case management, findings, or reporting.Design★ 281
- iterate-lessons-logproduct-on-purpose/pm-skillsCreates a structured lessons learned entry for organizational memory. Use after an incident, a completed project, or a significant learning to record knowledge for future teams and initiatives. Distinct from iterate-retrospective, which facilitates the team ceremony; this skill writes the durable lessons entry that outlives it.Writing★ 757
