writing-defect-reports
From the author
Establish a finding before you publish it, and correct it after — headlines that overstate what actually reproduces at the layer a user sees, reporting code that no entry point can reach or that is already dead, filing a caveat the project's own records already answered, re-verifying your prior notes against the tree instead of against the note, choosing the narrowest injection point that reproduces a failure without breaking the run first, capturing probe output a harness swallows, and withdrawing a published claim in the thread where you published it. Use when filing an issue, writing the body of a PR or code review that asserts a defect, triaging someone else's report, or deciding whether a suspicious observation is reportable at all.

- write-specanthropics/knowledge-work-pluginsWrite a feature spec or PRD from a problem statement or feature idea. Use when turning a vague idea or user request into a structured document, scoping a feature with goals and non-goals, defining success metrics and acceptance criteria, or breaking a big ask into a phased spec.OfficialWriting★ 31K
- correctmichael-denyer/pstack-claudeFind the mistakes agents keep repeating in this repo and make each one impossible. Try architecture first, then types, then a lint whose error names the fix, then a test, and write docs last. Prove each check fails on a real past mistake. Repeat this each time the operator corrects you. Use for /correct.Testing★ 3.2K
- write-a-postmorteminkeep/open-knowledgeWrite a blameless incident postmortem under postmortems/ following the Google SRE shape — evidence-based timeline, trigger vs root cause vs symptom, contributing factors, what went well, and owned+dated+verifiable action items. Read when asked to write a postmortem, do an incident review, run a root cause analysis, write up the outage, retro on the outage, or when the user says we had an incident and wants it documented. Do NOT read to frame a proposal (use frame-a-proposal), write a spec (use write-a-spec), record a decision (use record-a-decision), or review a design (use review-a-design) — a postmortem documents an incident that already happened, it does not propose, specify, decide, or critique future work.Writing★ 4.6K
- lcx-report-bugcode-yeongyu/oh-my-openagentCreate a high-signal bug issue or PR in the repo that owns the defect. Use this whenever the user asks to report, file, open, or triage a LazyCodex, lazycodex-ai, omo-codex, Codex plugin, or upstream Codex CLI bug, especially when they need source-backed root cause, reproduction steps, fix guidance, and GitHub routing.Engineering★ 70.2K
- code-reviewanthropics/knowledge-work-pluginsReview code changes for security, performance, and correctness. Trigger with a PR URL or diff, "review this before I merge", "is this code safe?", or when checking a change for N+1 queries, injection risks, missing edge cases, or error handling gaps.OfficialEngineering★ 31K
- write-a-specinkeep/open-knowledgeScope a feature end to end and write an implementation spec under specs/ from an accepted proposal — current-system mapping, goals/non-goals, a Decision Log for one-way-door choices, a live Open Questions backlog, and a real migration + test plan. Read when asked to write a spec, scope this feature, turn this proposal into a spec, plan the implementation, or break this into tasks. Do NOT fire on frame a proposal or write the PRD (sibling frame-a-proposal — a PRD frames a change before it is accepted; this skill starts once one is), record a decision or write the ADR (record-a-decision), write a postmortem (write-a-postmortem), or review this design (review-a-design) — those are separate skills. Complements the platform open-knowledge skill; does not replace it.Writing★ 4.6K
